By making the above payment, you agree to the following terms

1. The hospital payment must be made at least 10 days before your surgery.
2. After your initial consultation in Brazil, you need to wait at least 3 days before beginning your surgery.
3. You must pay for the surgery in full during your initial consultation in Brazil.
4. Failure to pay will result in a postponed surgery.
5. We always bill in R$ (Brazilian Reals). Final costs will be reflected by current currency conversion rates. For an estimate, please check www.xe.com. However, rates will ultimately be decided by your credit card company or bank.
6. It is your responsibility to alert your credit card company of upcoming travel plans. Making overseas purchases may result in a declined transaction.
